Registered Practical Nurse (RPN) – Community Care

Pioneer Home Health Care is seeking compassionate and skilled Registered Practical Nurses (RPNs) to provide high-quality, client-centered care in home and community settings.

In this community nursing role, you will carry out scheduled visits to support clients with varying levels of care needs. You will work closely with Registered Nurses (RNs) and other healthcare professionals to ensure safe, effective, and personalized care.

This role is ideal for nurses who value independence, flexibility, and the opportunity to build meaningful, long-term relationships with clients while supporting them to remain safely in their homes.

Key Responsibilities

As a Community RPN, you will:

  • Perform scheduled home visits to deliver nursing care within your scope of practice
  • Provide safe, ethical, and patient-centered care
  • Assess patient needs using clinical tools and professional judgment
  • Implement, monitor, and update individualized care plans
  • Administer medications and perform approved treatments
  • Provide wound care and chronic disease support when required
  • Build strong therapeutic relationships with clients and families
  • Maintain accurate and timely clinical documentation
  • Collaborate with RNs and the broader care team for complex cases
  • Ensure patient safety and report changes in condition promptly



Qualifications

Successful candidates must have:

  • Current registration with the College of Nurses of Ontario (CNO) as an RPN in good standing
  • Valid driver’s license and access to a reliable vehicle
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently in community settings
  • Compassionate and professional approach to care
  • Clear Vulnerable Sector Check
  • CPR/First Aid certification (required or willing to obtain)


Community or home care experience is considered an asset.
